Well Rarity has made some stretchy dresses before, so I’m sure they fit underneath the dress. Besides most of Canterlot’s population are unicorns, so it’s no surprise the dress doesn’t have holes for wings.
I don’t think Twilight had any say in the dress design this time; she was just modeling it.

Remember Twilight’s taste in dresses. That actually makes sense for Twilight. :-)
