For the record, my headcanon is that they are sisters. Even the ccg card leaves it open to interpretation: it only says that they are longtime friends with Thunderlane.

here’s the thing, the royal sisters occupy this niche that’s best described as power, both political and religious, we’ve seen the political side is their dealings with the general pony populous, and the religious side in certain ceremonies(weddings, coronations, etc). HERE we see the mayor, till now a solid political pony, performing a religious function, thus occupying the same niche, with the sisters in attendance and obviously with their blessing…………….fillies and gentlecolts, I give you the Equestrian Paladin.

Man, he didn’t even take Cadance with him. Their marriage is in trouble.

So the Doctor and Derpy were shown as friends but they also added Doctor and Rose (who seemed to not know him) to keep everyone as happy as possible.
