In short, a ferverish Applebloom gets kidnapped from Nurse Kindheart because the latter has the faint hope of training Diamond Tiara, her daughter, in the art of taking care of ponies.
 
So, Nurse Kindheart gives Diamond Tiara an entire hospital wing’s of supplies and machinery, and asks her to heal Applebloom. Since Diamond Tiara wants to do anything but heal Applebloom, she starts to torture her a bit, both psychologically and physically, with the machines and instruments lying around. The suffering leaes Applebloom incontinent, thusly7 causing Diamond Tiara to put salt upon the woudns by babying her.
