Twins do happen rarely in equines, but the births are extremely dangerous, and pregnancies resulting in twins are typically aborted by the mare’s owners.
Realism isn’t as much the issue as “Why are you doing this it looks totally weird” is.
Between FIM-verse ponies’ supernatural mojo, apparently unnatural durability (Derpy busting Town Hall apart with her rump, etc.), and already cartoony proportions? Yes, I’d say there’s ample reason to give exaggerated bumps a pass so long as ‘hyper’ territory (i.e. more belly than pony) hasn’t been hit. Hell, look at the elastic bodily/facial distortions Pinkie Pie demonstrates and tell me FIM cares one whit about ‘realistic’ limits.
That’s really not much of a point. Are you saying that an abnormally huge belly size is “justified” because the ponies do people things in the show? What a total non sequitur.

Yeah, but they were pretty tiny at birth. MLP ponies are proportioned very differently from normal ponies, but still. I dunno, I think soundtea is probably right.

FiM Ponies are rather small compared to real ponies. Also it depends on twins, triplets or what have you. Sure I don’t think I have ever heard of a horse having twins, but in MLP we have already confirmed twins.
