It is kinda both. It’s a Paintover, where they use a 3D rendering as a base but then heavily modify and stylize it in photoshop afterwards.

I’m always blown away by your art. This is 3D, and when it’s zoomed out that’s apparent, but when I click the image and it pulls up full res I find myself second guessing if this is a 3D render or a digital painting. Your work amazing, and I hope you continue to bless us with it for a long time.
Also, I have hated this meme almost since it started, but this one is extremely cute. Most of the pics in this meme are just excuses to draw butts, but this one feels legit just like a silly situation, which I prefer for the meme. All in all just a great pic here!
