I can’t speak for anyone else, but when I was a child I was so obsessed with being “manly” and “stoic”, that I dissociated whenever I felt sentimental, sensitive, weak, ashamed, or incompetent thinking that dissociation was stoicism. Diapers, as a strong symbol of shame and incompetence would also trigger dissociation for me. Just hearing someone say the word “diaper” turned my emotions off.
 
After suppressing these feelings for so long, they all became intermixed. Essentially I considered sentimentality to be just as shameful as wearing diapers, so by overcoming the inhibition to feel sentimental, I overcame them all the rest, diapers included. Thus, diapers came to symbolize and evoke all those other feelings I repressed as well.
 
tl;dr: childhood issues, for me at least.
