They’re pretty gross when dirty, kinda gross when wet and it’s someone else’s, a tiny bit gross when wet and it’s your own, and not gross when clean. But also the wetness can be hot. In both meanings of the word.
 
But when clean I find diapers less gross than normal underwear since they’re thicker and mean there’s a waterproof barrier between privates and the world.
 
In fact, I only lounge around without pants if I’m wearing a diaper for that exact reason. Putting your butt on the chair with nothing but typical underpants is gross. But a diaper is a far better barrier than even both layers of normal clothes.
 
So… diapers themselves aren’t gross. It’s what fills them.
