I would personally say mostly just considerably bitter than it is sweet, just another horrible reminder of how unfair life is and how you can do nothing wrong and still get screwed over.
I believe the usual way to feel about characters like Sugar Plum here would be called “bittersweet”.
As shown in this post, her disease makes it so that the right side of her face doesn’t work very well (presumably meaning she has little control over those muscles), and it causes her right foreleg to hurt (though it’s unclear if the pain is only if she does certain things, if it comes and goes, or if it’s just constant).
Plus her speech pattern suggests that her disease also either stunted her mental growth, caused her family to largely neglect her education, or both.
But in spite of all of these things, she remains adorably and admirably optimistic.
