This means by generously offering herself as a heat sink, she can allow her friend to gorge on sugar and not have to worry about overheating :P

The strain it puts on her mind and body to control that much extra mass and the energy and nutrients needed is disproportionate to the size itself :D
How can it be unhealthy if there’s no organs or blood vessels or nerves or bones or cells?
So, in more than one sense, they get thicker.
To an extent. They do get bigger by overeating, but it’s like the pony equivalent to obesity: the larger size makes it harder to move and they become a lot more lethargic and unhealthy :D
