When reading while walking, you don’t hold the book up in front of your face, because then you can’t see where you’re going. Just like browsing your phone, it’s down almost flat, and you’re looking down at it.
 
Twilight’s head is pointed that way because she just walked into the wall hard enough to embed her horn. That’s going to force her muzzle straight down from the impact.

that book was her true love. I imagine young twi was a super nerd and read a book 1 inch away from her face at all times

Am I the only one wondering just how this was supposed to happen? I mean, why would she be walking and reading with the book held so low, more or less parallel with the ground, with her head held in the same manner? This kind of thing can only happen when her horn is at a 90 degree angle perpendicular to the wall.
