It didn’t last that long, but another Anon had an interesting follow-up to OP’s theory, involving Equestria Girls and the “Human” Twilight.
It’s gone now, but it’s not like it had many repiles anyway
Or has the cast gotten that good at staying out of a hospital?
Basically that the Twilight we’ve been seeing since magical mystery mystery cure is really Nurse Redheart in such a convincing disguise that even she think she’s Twilight. Meanwhile the real Twilight is elsewhere doing something important.
Since the twilight image says “nurse-redheart”, maybe it’s trying to imply that Nurse Redheart became Twilicorn, which would explain why she hasn’t been seen ever since season two and hasn’t appeared in the comics since the 9th issue.
