Well, the guy in the image is Aerys Targaryen aka. The Mad King. The guy basically went completely mad, and I’m talking Caligula meets Nero levels: He had people burned alive from lords he didn’t like, anyone who was actually against him and was basically crazy. Eventually, naturally, the rest of the realm rebelled against him and after a time the war went in his enemies favor. As the enemies were storming the gates though his bodyguard, also the heir to a noble house that he kept as an “Insurance Policy”, stabbed him through the back as this was happening.
 
Now Targaryens were once Dragon Riders and raised Dragons until they died out, and it was believed that they had the blood of dragons by some in their line. Aerys was one of those, he believed that he would rise as a dragon upon his death…needless to say, that didn’t happen.
 
TLDR: I think this image is implying that Aerys’ madness was, well, him being able to see Twilight and Spike but nobody else could. Leading everyone to think he was crazy.

Twi has magic, though. I dun think they could stop her.

I never read/watched Game of Thrones, so yeah, went right over my head.
