The only reason pinkie doesn’t have diabeetus is because she’s pinkie pie, the excess sugar is probably just directly converted into pure energy to be stored in an extradimensional space for later use

I know of the fics you speak of, I’ve been in the fandom for over 7 years.
That reminds me, back during season one there were two famous fanfics involving Pinkie Pie. Cupcakes and… Bittersweet. The latter was a fanfic where Pinkie gets diabetes and dies.
In the years since then, I don’t think as many people remember Bittersweet though.
I’m sure even a candy mare like Bon Bon knows to practice proper hygiene. That’s like saying that Pinkie should’ve had diabetes since Season 1.
