As stated before, Sombra is the husband of Iron Will and Fluttershy. They are a loving group and are pinnacles of their community. That doesn’t mean their relationship is without bumps. Sombra sometimes has relapses and has to be restrained for a few hours so he doesn’t hurt anyone. It starts out with him talking to himself and seemingly in tongues and then leads to fits of unbridled rage. He coughs up an inky substance and bleeds from his eyes at the last stages.
 
In this instance, it’s the first time he relapses upon awakening from his ‘I got blown up with Crystal empire magic’. He was brought back by Iron and while Flutters was wary, she felt she had to care for him since he was injured. Then he woke up screaming bloody murder and attempting to escape. To be fair, he did only just finish regenerating himself after being blown up, so him freaking out is a given.
