Even though Twilight is an alicorn, she isn’t as strong as Rainbow Dash or AJ.
She hit him so hard that she forced his ankle to bend slightly outwards. And remember Twilight’s an alicorn, hence has an earth pony’s strength.
Imagine if a rather buff guy kicked the back of your ankle hard enough to genuinely bash it in like this. No matter how tough you think you are, it’s gonna hurt like hell.
Twilight actually did this later. And he even put his hoof on her flank for a moment :)
