Indeed, it had a promising start.

I also suspect it has more something to do with magic, that there is a sensibility in the first place. This is also just an equestrian unicorn/alicorn thing.
The way I see it, the horn acts as a conduit for channeling magic, to ensure a constant flow/connection in tandem with the environmental magic. By touching the horn with something… eh… warm and soft (in this case wet too), receptors, originally meant to be used to ensure sensibility to magic, that are naturally woven into the horn, are triggered, in a way that is causing a pleasurable feeling which is transferred directly into the brain of the said specimen.
Maybe the horn (the extra limb) isn’t as much pleasured as the brain that receives the send signals and interprets them as something pleasurable. I may also have to do something with the heat that the oral organ sends over the horn, the temperature change suddenly inducing a signal to the horn that is equal to pleasure.
Otherwise, the kind of pleasure that they interpret of this, maybe equal to excitement and embarrassment, causing in context with a social norm in regards to “somepony else ‘softly’ touching the horn of another”, a sort of ‘taboo effect’ feeling, which then in turn makes them feel aroused, because you know, something naughty is done to their body.
Attacking the horn, stuns them or even seriously mentally renders them unable to stay conscious, as well as overusing magic giving them a serious headache. All of this speaks for the results of malicious and careless use of their horns, so it is very likely that a ‘tender loving care’ of their horns may trigger the exact opposite, instead of causing pain to their brain, stimulating it.
Although in this case here, I suspect that Molly Daybraker cheated, because she somehow infused magic into the Guard Pony, or tempered with his ‘magic conduit’ piece, while ‘pleasuring his horn’ intensifying the results, both the positive feeling of pleasure and relief of pain.
Just have to accept that their horns aren’t simply bony means of defense, like those of “real deer” for example, but sensitive parts that act as a conduit to magic, their own and environmental, there for tempering with them can be much more of a distraction in focus and lead to all sorts of feelings.
