I pretty sure it’s just the fact that his muzzle looks like it was bashed in by a train. For some reason, I’ve never found normal Zen to creepy; though he does look like a old man who just tasted something really sour
If you look at the actual Zenyatta he’s somewhat unsettling too
Pretty sure it’s something inherent in the nature of him being a sapient machine that has the basic shape of a human but is otherwise clearly not one.
Like the face, it’s just a couple of slots for what I assume are optics, some lights that may or may not serve some purpose, and other than that it’s just a faceplate. As in, literally a couple of peices of metal/plasic to cover up whatever is in his head.
