That would make an interesting story. maybe they had a genetic disease that was linked to the Y chromosome, so the only solution was to genderswap them as to get rid of said Y chromosome. I can imagine dash in future going through her baby pictures and being like “wait I minute? Is that…MOM, DAD!!!”
I mean in the case of Dash

Hence why I said “I doubt” regarding their surgical skills, I hope I didn’t imply the show stated it one way or another. We know they can do surgeries of some sort, but chances are we won’t ever know if they can do those kind of surgeries.

it was never stated that their technology and surgeries aren’t evolved enough.
A lack of technology that doesn’t rely on magic, and an overall lack of a need for it.
Why?
Indeed, we have too small a sample size of baby ponies to really tell if its a rule that they all look like that at birth depending on their sex. The adults as you noted show there is substantial variation in muzzles and eyelashes.
Plus we know changing a pony’s gender via magic is considered all but impossible, and I doubt their surgical skills are on par with the real world.
And of course we all know animations errors are nothing new for the series.

Maybe she transitioned at a really early age and that’s a contributing factor as to why her parents are so excitable.
