Actually once read a fic involing that happening under… special circunstances.
 
Well, if you look at Pumpkin Cake, her horn is really tiny. I imagine that between that, and that only Alicorns seem to have actually pointy horns (while most others end on a stump-like end, which I’d expect to be even rounder with babies), it doesn’t seem like it would be THAT much of a problem.
 
The least be said about motherfucking Flurry Heart, though, the better…

The idea I had before reading the description was that her “sister” was always her daughter. Which of course puts the Nightmare Moon thing in a darker light.

While I know this isn’t what’s intended from the pic, I kinda like the idea of unbirth scenarios used for healing. Like, for example, this being a process the sisters underwent in order to restore Luna from her Season 1 self to her Season 2 upgrade.
