The examples you listed are not unusual, most uploaders are lazy with their tagging and sometimes some obvious features that needs to be tagged are not getting tagged at all. I also think that most people are not as careful with their filters as you’d think they are.

I notice them mostly on grimdark tag images (mostly people just need to use filters, smashing the Downvote button because their precious pony is suffering, they think it’s all cringe, or simply dislike gore).
I also see it in most crossovers, but this surprises me because a lot of bronies are TF2 players/fans, at least I thought.
