And yet sometimes they still slap in on the cover in the foreground. I remember one comic was just Logan taking a fishing trip and all these superheroes keep bugging him for a team-up.

It’s better than in the Silver Age where they made covers first and then told the writers “Make the story like this”.
Go look at Superdickery and wonder just what kind of stories could be told with covers like that. Especially since the stories had to end with the status quo and the cover shows the Earth blowing up.
MLP is nothing new. Just look at wolverine; he shows up on Xmen comic covers when he never appears inside.
