Inspired by the Force Ghosts in Star Wars, and by the idea that spirits can visit those they knew in life, or even become their guardian angels. Rainbow is almost certainly the spirit protector of Scootaloo.
As for how she died, I would imagine that it was heroic, a selfless sacrifice in defense of her friends. RD wouldn’t have it any other way I should think.
