Nope. That is “Rarity” but it’s not an age regressed version but an actual newborn that just resembles her. In my fic SB gets a new sister near the end.
 
This is one of the happier moments of the fic, but the earlier parts are kinda sad. Rarity comes down with an uncurable disease, and SB turns to Discord in desperation. And what he wants in return would put all of Equestria at risk; an element of harmony.

Hey there. I was the one who commissioned this pic and I want to point out the tags are incorrect. The pony SB is holding is an OC. SB gets a new sister in my fanfic that just happens to resemble Rarity, and she is given the name of her older sister because of it, but it’s not age regression or role reversal. I get why you thought it was that though.
