I think King Sombra was dead a long time ago. The one we see is the shadow of its former self.  
I know, this headcanon sound unoriginal.

The idea that Sombra can “regenerate” from a lousy horn is totally ridiculous.

New threat imprisons everyone except spike who twilight teleported at the last second, spike wanders around in the tundra aimlessly until he collapses, when collapsed he is called by sombra’s horn,  
“Child of my enemy, why have you come? I offer no forgiveness, a mother’s sins, passed to her son.”  
After dialogue explaining what happens, Sombra all like  
“do you hate them?”  
“……”  
“DO YOU HATE THEM?”  
“….yes….”  
“then join your body with my mind, whelp, and their souls will be for the taking.”

Sombra always seemed like a shadowy demi-lich, trying to reach his phylactery, the crystal heart… which is why he became flesh and blood again when he approached it, but still undead.

To me “imprisoned in hell” seems like a fate worse than death :o
