As CrispiSalad said on the previous panel, this was possibly intentional, and almost certainly a good thing on Izzy’s part. I only put that “almost” in there because it could backfire a bit, but this really seems like the safer bet.
Point being, I strongly suspect that the next panel will be Izzy explaining “No, the bracelet is the correct size, your fetlock ((wrist?)) needs to be thicker!” and if it isn’t and Izzy is about to apologize I expect somepony else to make the point instead.
… of course it now occurs to me that she might be having issues with the weight which would be… hmm… not nearly as good of a way of making the point, and would almost certainly not intentional on Izzy’s part, but could serve as the same sort of reinforcement if spun right? Not being able to comfortably have it on (rather than just having to keep PUTTING it on for a diameter problem) could be demoralizing in a “I can’t even enjoy this present and that is insulting to Izzy! I must be a bad friend!” sort of way. Which, of course, would be insane depression (?) logic.

Since the hospital has heating (although set low for the benefit of the staff who have to wear protective gear) she might not need it, and they might need access to her arms and torso(1)
(1)For instance to notice bruising from Izzy’s hug, or MAYBE even for a visual (or tactile?) clue to if she has been purging(2) or if someone didn’t get the memo about “feeding her too much now could harm or even kill her”.
(2) But appropriate portion size to avoid refeeding syndrome or whatever that is called might trump “really thin” especially for visual stuff?
