This also could be a simple bloat. Mares are very self-conscious abouts bloats.

It’s a fantasy world of multi-colored talking ponies. I doubt there’d be any serious consequences from being fat other than that they’d move more slowly(and Octavia is hardly what one would consider morbidly obese to begin with).
Yes being morbidly obese and immobile and at high risk of having a heart attack is “cute”
