The intro mentions Snake being injected with various drugs, ostensibly to help him survive the arctic conditions. Then people keep randomly dying when they meet him. Turns out it's because his pre-mission injections was a engineered virus.
The real ex machina is how it doesn't kill Snake at all. But later games explained that with their go-to BS (Nanomachines, son).
