He appears randomly. Don’t look at him directly, and if you see his hand, pull up the camera.
I think if he appears during your playthrough you should look to the left until you see his arm then go to back to the camera and go out and he shouldnt be there.

Not exactly. Its all about just Fnaf3. You get hallucinations like Foxy being this. They look burnt because of a certain ending in the game.
