Ah ok, it looked like a Stallion so I was a bit kerfuzzled.
It’s an alternate pip, the same as these.

I’m not the tagger, but I’m pretty sure that the Unicorn in the center is meant to be Lil’ Pip. The artist’s style and slight palette-slide might be tripping us up.
I always figured that if you took the Wild Wasteland perk the various zany, otherworldly events were actually happening, which suits me just fine seeing as how I played my Courier as a 1 Intelligence would-be superheroine roaming the Mojave Wasteland in a bright red spacesuit and carrying Euclid’s C-Finder.
