I think this is actually the reason they made him a dog in Equestria Girls. If they made him a little boy, it would have made the slapstick look a lot more like child abuse.
I just happened to stumble on this, too:

Ignoring the sexual connotations in our wording, yeah its still pretty messed up. That kid was in serious pain from all that reverse fire-breath spells. While Twilight as all cheery that she got inspiration from her wise and powerful mentor, Spike was sick as a ….goddamit hasbro. I can’t even use the phrase ‘sick as a dog’ due to how shitty you treat Spike’s character most of the time.
But yeah, Spike was sick as a dog at the time.
That was kinda messed up for Spike, and he’s a minor for goodness’ sake!

Hell, the resolution to Return to Harmony proves it…stuffing letters down Spike’s throat to Twilight to tell her to save the day….after Twilight herself had given up.
Not to toot my own horn, but I prefer the idea of Twilight realizing how little control she’s had over her own life and flipping the fuck out.
