Diapers only have two uses. Receptacles of bodily waste, and fetishism, whether related to using them as intended, infantilism, or just as a fetish object in and of themselves. Feet are for walking on, and are a part of the body. If someone’s sticking their feet up at the camera in a “look at my sexy feet” kind of way, yeah, that should be tagged as suggestive.
 
If something is intended to be fetish fuel, it shouldn’t be marked safe. If it clearly is not, it should be, regardless of whether or not elements of it can be seen as attractive by people with specific fetishes. This image can get away with being marked Safe only because Scootaloo is still a young filly. If it was a fullgrown mare/stallion, there would be no question.

I guess after Rainbow Dash ditched the Scootaloser in the last installment, she needed something to raise her spirits. Well, at least we have something to laugh at!

Sadly there are that awful kind of people who masturbate to foals. Also there are people who’d do it to a picture that would look totally safe and innocent to most people. Its not automatically questionable, only when its being worn by someone other than babies/newborn foals, because outside of excuses like “but its medically required” its normally because someone is supposed to be masturbating to it. (And even the flimsy excuses are almost always just ways to get them into situations for people to jerk off to.)
