Derpy was both an animation error, and not. Let me explain…
 
From what I understand, an animator of that crowd scene in Ep1 derped the pegasus’s eyes as a sort of in-joke, intending it to be covered up by another row of ponies, or at least fixed in post-production.
 
However, Lauren or Jayson or Wootie or someone saw the rough cut of that scene and thought there were too many ponies in the room, and had that front row taken away, leaving the derped Pegasus exposed. At that point, they either didn’t notice her or maybe decided to keep her in just for the hell of it (I think Jayson might have said this).
 
We all know what happened next. Derpy became a thing on 4chan and spread like wildfire. The staff noticed and quickly moved to insert her – with derped eyes – into as many random scenes as they could, starting with the earliest ep still in-production at the time, “Feeling Pinkie Keen”.
 
FiM probably would’ve still earned a fanbase even without The Derped One, but I wonder how/if the fanbase might have turned out differently without our unofficial mascot.

Derpy actually WAS an error - it was an animation mistake in an early episode that gave her crossed eyes. The animation team noticed how much everyone loved her and started putting her in on purpose after that.
 
And Slowpoke isn’t an error, he’s just associated with Princess Erroria because this sudden burst of interest in her was started when someone on 4chan noticed “hey guise, there’s an alicorn in this shot!” … in an episode from TWO YEARS AGO. Everyone knew about her already, although very little had actually been done with her character since then.
 
Because of that, she’s often seen riding or with a slowpoke.

Why is Slowpoke an error? And why is Derpy an error too?

Fun fact: The I am error guy from Zelda II is actually intentionally named “Error”, and has a brother whose name is supposed to be “Bug”. However, the brother’s name was accidentally left untranslated from the original Japanese “Bagu” in the U.S. version, making him the (developer) error in the game.

My headcanon has it that Noteworthy is one of Carrot’s EXes. I always figured Carrot to be a bit of a scrooge when it comes to Bits, which fits with her name Golden Harvest, so she tries to have as much disposable income as possible. Hence why she rents her spare bedrooms to Derpy and Dinky, and further in my headcanon, Amethyst Star.  
Noteworthy is a struggling musician, so he got dumped. Written Script is a successful writer, so Carrot’s had a longer relationship with him, to Noteworthy’s dismay.

You could totally ship Derpy/Noteworthy.
 
“Noteworthy” suggests that he’s a pony who tries hard for recognition but remains a background pony.  
He lusts after Carrot Top in bg scenes but we know that Carrot is fooling around with that other Written Script colt.  
So with all that and their eye errors they’d already have a lot in common.
