They absolutely do, as well as experience stress. All the behavioral studies are consistent on that front. They react to painful stimuli exactly the way we would expect if it was an unpleasant sensation that affects their well-being.

Gotta say, between the kinda awkward flirting from spike and the unsure expression from lily in the last pannel this page has a really weird energy to me like it’s supposed to look like a cute outing but it feels kinda awkward?  
(Now I understand if lily’s expression is in reference to the “do we eat them” line but the rest of the dialogue and spikes face have a more positive ring so it seems like an awkward choice to end on)
