Because he inadvertently wished on a star that made you obsessed with spending so much time with him. That caused him to eventually realise he was doing the same to you and that it can be quite aggravating when you are on the receiving end.  
Don’t worry, he doesn’t really hate you for it.

I thought the point of the pic was that she has no special somepony at all, and the closest to a gift was the Ruby Spike gave her (no romance implied)

I see dead ponies...
The gem seems to be a clue as to what she’s thinking about. Who gave her that gem? Spike and if we are to assume they are together then this must be their first hearts and hooves day since then apart.
