If those public toilets are like the ones I’ve been forced to use sometimes, the forest would probably be cleaner and less of a health risk to use.

It’s not without precedent in FiM. And at least no one wets themselves (onscreen, I think Fleur might not make it). For some reason, the edited version of Dragon Ball did not cut out Bulma wetting herself onscreen.
 
And trust me, this is a fetish to some people. Desperation for some, wetting for others.

Surely they would have enough self-awareness to be able to understand that their fetishes are uncommon and that there’s a difference between intentional fetish content and content that just happens to appeal to their fetish. I’m a vorarephile, but I never confuse scenes of characters getting eaten for intentional fetish content.

I can answer this. Fetishes.Plain and simple.
 
Certain scenes that my seem innocent or not sexual to any other person may seem entirely different to some one with that fetish. I.E. This scene of having to use the bathroom really badly, or for another example, that one scene with fluttershy exposing her bare feet in the pool.
