The voting system on Derpibooru, such as it is, isn’t very fine-grained. There’s four options: favorite, upvote, nothing, and downvote. That doesn’t leave room for distinguishing between “I really like this, even though it’s pretty simple and basic” and “Great googly moogly, this is awesome!”. So yeah, a drawing like this that appeals to everyone with artist:lumineko, coco pommel, and cute in their watchlists is going to draw a lot of upvotes, putting it on the front page where it can get more upvotes, while a really intricate, scenery porn-type drawing by a less-prolific artist with a minor background pony will never get the attention it needs to get a comparable number of upvotes. That’s just the way it is.

People like it because it’s a cute picture (1) of a popular character (2) in a vulnerable situation (3), just like #36F7 said. This has been explained to you. If you think it’s overrated, that’s fine, but don’t antagonize people who do like it and have upvoted it.

pffft calm down you coco-obsession, the pic its just a redraw of a scene rushed and little stylized, I understand Lumi isn’t the super-detail sort artist, that’s ok I have faved his work many times too, but it’s just dumb the fact people fave so much a low quality pic like this just because coco is on it, it could be made of shit and vomit and you still fave it just because coco is in the pic. And no I don’t hate coco, but up-fave idiotically it’s just annoying.

Yeah, it’s getting upvoted and faved into the stratosphere because it’s the uber-moe waifu being all sick and in need of pampering, which is pretty much the perfect storm of brony bait, but it’s a nicely done, cute drawing that effectively uses a slightly rough, sketchy style to enliven a simple portrait. It’s not really that different from the similarly-skillful drawings that rocket to the top of the three-day scoring display because bronies watch it for the plot.

I don’t understand what’s so special about this pic it looks sketchy and unfinished, but hey it’s coco pommel I must fave :/

Probably the same instinct that drives many to irrationally defend the weak or helpless.
 
Personally, I suspect it’s largely derived from the primal instincts that encourages us to preserve our species, which includes helping a sick individual recover faster. Plus, by helping a vulnerable individual, others will be more likely to perceive the helper as ‘friend’ or ‘ally’, thus improving the helper’s odds of survival, since they’ll make less enemies this way.
 
It makes sense to me, at any rate.
