It ends a little soon to be sure, but I don’t think this is Twilight being an insensitive pervert. I think its totally the opposite.
 
Flash bemoans the fact he can’t please a mare anymore, since an injury seems to have rendered him neuter. Thats why he hasn’t taken Twilight up on her offer. Twilight is saying she doesn’t care he doesn’t have a dick, there’s still other stuff they can do together. She’s still interested in him despite his injury. He can still please this mare.

I think most guys in his situation would prefer to have some kind of physical relationship with a woman. Throwing up their hands (hooves?) and saying ‘oh well, never doing physical intimacy again in my life’ is the ultimate surrender to despair.
