The fact of the matter is that no matter what way this happens, injury is imminent. While getting one’s teeth knocked out would be a major concern for a straighforward buttgrab, simply going around and grabbing her by her midsection in order to pull her out might not necessarily be worth the pain dealt out afterwards. I believe that the best way to balance the risk/reward factor involved in pulling her out of this rock would be to initiate the action as you suggested, by going around and grabbing her by her midsection and then beginning to pull her out. However, while working to free her from the rock, one should begin to sneak their hand towards the tail end of her body. While the injury afterwards might be nominally worse on account of how she would feel the hand slowly sliding upwards, the primary objective would still be obtained and I firmly believe that the reward would outweigh the cost of the numerous bruises and lacerations that would happen after she had been set free.
