It wasn’t so much a leak but last week it aired in Canada on Treehouse TV.
This episode actually comes before CotLM in production order and was originally suppose to air before it as well. It was pushed back so they could air it on Halloween.
That’s probably why it was accidentally released. The copy sent to iTunes still had the originally air date.

You are right about the first mess up where it was out on iTunes in September, but the second incident not so much, that was last week and it was three weeks ago they got them.
