Or maybe two different groups worked on the same character at the same time. without contacting one another for the notes

From a certain point of view, the different cutie marks might not be the fault of discontinuity. From behind the scenes, Andy was given a description of Fleetfoot’s cutie mark and he drew it as he imagined it from the description. So the marks aren’t so much different but rather different versions of the same mark. Eye colors, however, can be the result of a miscommunication or just plain oversight.
